As a new student employee at Bluffton University, you need to complete various tax forms in order to get paid. You will find four different forms linked below. Print, complete and deliver all completed forms and two original forms of ID to the business office receptionist, located in the lower level of College Hall (Please note: photo copies are not acceptable).
Ohio Department of Taxation Form - Employee’s Withholding Exemption Certificate
If you live in Ohio and you do not know the school district number, the business office can furnish. For this question, please note it applies to the public school district where you live which might be different than the school district you attended. You may claim 1 or 0 on Line 5. If you live in Indiana, Kentucky, W. Virginia, Michigan or Pennsylvania, you may claim exemption from paying Ohio tax by filling out a certificate of reciprocity form, which can be obtained and completed in the business office.
Public School District Withholding Certificate
This form needs to be filled out if you live in Ohio. The public school district number is available in business office. Students who reside outside of Ohio do not need to complete or return this form.
W-4 Form - Federal Withholding Allowance Certificate
You may claim 1 or 0 on Line 5. If exempt is used on Line 7, this form must be filled out each year (January) that you work at Bluffton University.
I-9 Form
Two forms of ID are required. A driver’s license and a social security card are used most often. Other ID’s may be used as listed on the back of form. You must bring original ID’s to the business office with your I-9 form.
Bring all completed forms to the business office receptionist, located in the lower level of College Hall. Enter the door to the right of the cashier’s window. Office hours for these forms are 8 a.m. – 12 Noon, and 1 – 5 p.m. As mentioned above, please note that the business office cannot issue a check until all appropriate forms are completed and on file.
